No, I have no idea who made the close mount kit. It's extremely cheap and flimsy, yet strong enough to hold a stack motor fan. I cant remember what fan I got it on, but it clearly wasnt original, the fan was AB and the kit is white.
It's basically an LCA made out of sheet metal instead of cast, and screws into a 1/2" threaded downrod flange.

BTW the ceiling in there is more than tall enough for a downrod and bell canopy (the floor slants down, there's a drain by the washer and dryer, so it's at least 9' where the fan is). I thought the close mount would be a good idea to best clear the shelves.
If there were no shelves, I'd install my 40" Dayton Lasko industrial on a short downrod. But we need the shelves for storage.

Oh, my 42" round vent Emerson came on one of those mounts, just assumed it was an Emerson offering. Wonder who did offer those.

Emerson sold a mount that looked very similar to mine, but it was a little more substantial.

Like I said on DT, mine looks even flimsier lol! My "hatch" takes up a full quarter of the round. I'm still in Minneapolis but can snap a pic when I get home. Not guaranteed I want to get rid of it, it's quite handy despite how awful it is in design.

The Youtube requests to fix things are crap, I get them for detection of shaky video and all it does there is merge frames together so it looks like a bad stop animation lol. I think you're fine on this one but don't even bother with it in future. Lovely to see things in motion, BTW.
