From Lewisville, TX... The faceplate (with GE vents) and brackets have ornate scrollwork on them like an Evergo once sold at Montgomery Ward. Never seen a 6 blade model though, nor have I ever seen such a gigantic switch housing except maybe on one of those old Codeps or an antique fan. There was a similar fan in the back of the store with 4 blades, but I couldn't get a clear picture of it. It looked like they had been replaced with builderfan blades. The storefront had a "for lease" sign in front of it, but I forgot to jot down the number. Perhaps I'll go back when I get some free time.

Looks like it might have been made for JCPenney . . . however I've never seen any ornate GE vent 6 blades before, nor any ornate and/or 6 blades with that canopy. The top part of the canopy looks Moss-ish. The whole thing is a mystery. It's a really cool fan.
