Need to ID this ceiling fan

Hello.  Sorry to be a bother but I can't identify this ceiling fan.  Description of second photo: Note that the blades are affixed to the blade holders using a knob which is on the opposite side of the blade.  Note 4 fixtures for lights.   Description of third photo:  Note slots for air cooling which are co-circumferential with the body of the motor assembly.  On the side of the light fixtures there is a matrix of perforations ; that is what the apparent square is on the sides of the light fixtures.  There are two pull chains.  One to the side of the part bearing the light fixtures and one emanating from the center of the fixture.  Any help on this would be greatly appreciated.
