Square metal motor housing
Flush mount to ceiling
4 blades
4 lights in a cross type setting with 90 deg turn bends for bulbs and globes
chrome or stainless steel motor housing and blade mounting hardware

Based on this description stated, this appears to possibly be a Catalina Lighting fan.
