The closest thing I can assume this is to is a Litex ceiling fan or something close to SMC. Not exactly sure what the exact name of the ceiling fan is.

Hmm...Wal-Mart comes to mind. It is worth noting that the UL file number is no longer valid, and therefore we likely cannot determine the exact manufacturer, however.

Why/when does a UL file number become invalid? When a factory either closes or changes testing laboratories.

It's not a Litex or Encon. But blades and arms are easy to find, any generic 52" blade and blade arm set will work with the fan.
