I only saw this one for a short while when I was young. Most of the details are hazy, some may be wrong.
-36"-42" fan
-Three piece motor housing, smooth sideband. Top housing piece had some sort of design, possibly like a Casa Victorian or Lady Delta
-No vent holes on the bottom, I believe the bottom was smooth. It may have had some sort of pattern
-I am 75% sure it had a flywheel motor.
-Four blades, I believe they had a rather steep pitch. I didnt see the blade brackets or blade tips but the blades mounted in a standard fashion
-The switch housing was wide but short and I believe it may have tapered smaller towards the bottom, similar to the switch housing on a Dayton-Lasko industrial or some Minkas from the 90s
-Two pullchains. I do not believe it had a variable speed control
-Small bell canopy similar to that of a Nutone or Key Largo
-Antique brass, wood blades
When I saw it it was running on high on a very hot day and it moved an incredible amount of air. It was hanging on a very long downrod, the ceiling was 20' or so but the fan hung down to floor level. It was over the counter in the gift shop of the Milwaukee Museum of Something before they remodeled.
Any ideas?
