Looks like a View Fan. They were also labeled Builders Square I believe. Target sold these as well in the 80s and early 90s. I believe Cole has a hugger of this style. They're cheaply-made, for sure.

More sightings:
-A diner had about 8 Casa Four Seasons II or III 42" motors in AB with a 38" blade span each. Out of all of them, one wobbled badly, two were on LCA's, one had a dropped flywheel, and three were off, but the two over our table were both running. They all had Hang-Trus as well. When did they stop making the 38" FS II?
-A hotel I stayed in had some kind of AB fans, not sure what kind as the lights were off as the restaurant wasn't yet open for the day when I saw it, but in the pub there were 2 or 3 AB fans of some sort, they could be older, as the hotel was really old but remodeled, so I'm not sure what kind they were.

Yep, same idea, NIB labeled Target, but with a View Fan warranty card. They're actually not that bad, the bearings just suck, as usual with View Fan. Noisy right out of the box...

I have more that I either remember or have seen recently: 1.) Just recently, my local Famous Dave's installed 3 Black outdoor CF's on the patio area where Uno's used to have 3 Weathered Brick Hunter Sea Air's. The Black 4B Hunter Classic Originals in the bar still remain. 2.) Also just recently, on a road trip, I ate at a Burger King in PA that had been remodeled in the early 2000s, and was most likely built in the early 1990s due to the BK in my area was built in the early 1990s and has the same exterior style. Anyways, there were 6 Chrome Casa Delta II's/III's with 5 High-Gloss White blades, all running on Medium. 3.) I remember a Wendy's on the Rt. 50 bridge in Ocean City, MD (in the same shopping center as a newly-remodeled (2008) SuperFresh. They had 2 or 4 PB Casa Delta II's with 4 Walnut blades. I think they were removed in 2006-7. They both had schoolhouse lights.* 4.) I remember another Wendy's that had an AB Casa Delta I 3-Speed with 4 Walnut blades, as well as an AB vari-speed K-63 Casa Four Seasons I with 4 Walnut/Cane blades. Both were running on Medium speed and had schoolhouse lights.* More restaurants in Ocean City, MD: There's a chain of pizza places in Ocean City, MD called The Dough Roller (really good food), and they have some cool CF's: -The one on 3RD Street had a bunch of Casas. Now there's a single AB Casa Delta II or III 5B with 5 Walnut blades left over from the remodel in 2005, as well as 2 3-Speed Casa Panama II's that were installed in 2006-7 and in PB with 5 Walnut blades. They also had a PB Fasco Gillespie with Cherry blades that got a dropped flywheel, and was replaced by a PB/AB HB Cairrage House II. All fans have been replaced by White Progress Lighting fans, and the Delta II was removed. -The one on 2ND Street had a bunch of older CF's (Casa Delta II's/III's, Evergo GEV Top-Mount Spinners, Emerson 52" Northwinds, Hunter Embassies, Hunter Quiet Breeze II's, and others), but it burned down in 2009 and was rebuilt. Now there are several Casa Ventura II IT's in PB with Cherry blades. -The one on 41ST Street has some PB Casa Ventura II IT's, and 2 Burnished Brass Hunter Covent Garden's.* -There was one on 100TH-something Street that closed and was turned into a crabhouse. They had some White Quorum GEV 52" builders and some PB Casa IT Venturas. Not sure if they still remain.* -A place called Phillip's Crabhouse on 21ST Street had some HB Grayton I's outside, later replaced by White 42" Outdoor Builderfans. In the stairwell entrance for the buffet area, they have a 52" Gloss White early Hunter Infiniti and a White 52" Minka Aire Contractor's Series. There were most likely 2 Hunter Infinities installed there at one time. There are 4 generic PB GEV 4B 52" Builders over the buffet salad bar area. The carry-out area had some ORB Emerson Heartwicks. -A place called JR's Ribs had some AB NuTone Verandah II's, but they were replaced with Brushed Nickel HB Classics recently. They had 4 1ST-Gen Rust Fasco Charleston 52"'s in the dining area, 2 were replaced by ORB HB Cameron II's. There are also mixed GEV Broans and builder vent generic CF's throughout the restaurant. -A place called Layton's Donuts has 2 Brushed Steel Emerson Maui Bays indoors, and some White Regency Outdoor Builders on the patio area. -A place called MR Ducks (a store that has restaurants) had a GEV Moss Heirloom that was replaced by a White HB Farmington. They also have some Bronze HB Palm Beaches, and a Brushed Nickel 44" early Hunter Stratford II.* -A place called Tony's Pizza had White Fasco swirl vent builders and Quorum Classic 52" GEV's in White throughtout, many have been abused, and some have been replaced by many types of CF's, HB 60" Industrials in White, White HB Farmingtons, White HB Quick Connects, and AB HB Landmarks that had the tiffany lights from the old fans. Some Quorum Classic 52"'s and 52" swirl vent fans still remain.* - A place called Boardwalk Joe's had 2 AB Huggers in the rear dining area, and some AB 42" GEV 5B Close-Mounts in the front dining area. The Huggers were replaced by AB Hunter LP III's (Home Depot Plus Versions) (2 others were installed), and the Huggers were moved to the front. One of the Huggers broke, and was replaced by a PB HB Rockport. Both fans in the front were replaced by Coke motif HB Littletons (SMC-made versions). Said place turned into a coffee bar, said fans are gone.-A place called Shennanigan's had some early White Emerson 52" Summer Nights outdoors (3 of them), one was taken down, the 2 others were replaced by White Hunter Mariners. Inside, there were 4 52" AB SMC Emperors and 4 AB SMC Emperor Huggers. The Huggers were replaced by 44" Bright Brass Hunter LP III's in 2003-4, the down rod Emperors were replaced by PB HB 52" Cheshires without lights, then replaced by Home Depot's Version of White 42" Hunter Low Profiles. The management office had a single AB Encon Crusader. Alright, so here's some updates on the Ocean City, MD fans, as I just got back after vacation:
*An update on JR's exact CF's: some AB Broan 52" GEV Builders, some AB Emerson Builder-Vent Builders, a 90s AB 52" Emerson Northwind, 2 HB Brushed Nickel Classic Style 52", 2 52" ORB HB Cameron II's, and 2 Rust 1ST-Gen Fasco Model 452 Chalestons. All of these CF's were there before. *Tony's Pizza has installed more HB 60" Industrials in White, the other CF's still remain. *Harrison's Harborwatch put in some AB Gulf Coast Monarchs in place of the Fascos. *The place with the HB Builder's Bests was turned into a taco place, and they were all replaced with ORB HB Cairrage House II's.
I'll upload pix l8r.

Here are some more recent restaurant CF sightings:
*The Ruby Tuesday in the mall 15 minutes away from me that used to have the Emersons went out of business and was replaced by a Greene Turtle, with 3 Minka Aire 52" Sundowners in Bronze outside, and several Bronze 68" HB Alturas. *A Red Robin took the place of an old Fuddrucker's in the same mall as the mall above. There are several Copper Modern Fan Company/Period Arts Fan Company Industry fans inside the restaurant. *I have been going to eat at a new restaurant that opened in 2007. It was originally built in 2006 as a crabhouse that wasn't very successful. An Italian restaurant bought the space in 2009, after it was vacant for three years. This restaurant had several 42" White Broan Decorator Series fans over every table. Broan apparantly switched to Casablanca-style blade brackets and the original installers of these fans bought some fans with pretzel brackets and some fans with Casablanca-style brackets. Some fans had both types of blade brackets, it was weird. Also in the bar there are three Canarm 56" Industrials. All fans were in a cool setting with a wood ceiling. The owner of the restaurant decided to remodel and change the restaurant's name. The Canarm 56" Industrials were replaced by eight Harbor Breeze Oil-Rubbed Bronze 52" Tilghmans with matching light kits. The Broans were replaced by Oil-Rubbed Bronze Hampton Bay Glendales.

Local (Seattle, etc.) Red Robin restaurants used to all have PB Casablanca Victorians with oak blades set in recessed square sections of the ceiling tiled with mirrors. They have loop-arm lights, but rather than the Casa ring fitter, the arms go into a very plain round box mounted under the switch housing. Over the front door, the same mirrored box would contain a Columbia II. These are getting replaced by the copper Blenderfan copies, but I'm trying to get my hands on a Columbia II. Send me your hopes, guys - one place took my name, but apparently lost it and the fans went in the dumpster!
A large Mexican restaurant used to have Casablanca Victorians, white/brass with cane blades, loop-arm lights with center shade. Some have been replaced, some are still there. On very long (maybe 10-12 feet) downrods. The lobby has a bunch of ornate brass wall sconces, many of them of varying designs, and I'm going to try to get my hands on one of them by offering to replace it: this one (and there's only one) consists of... Imagine the OLD Casa ornate hubs, two ornate caps sandwiching a ring with light arms. Now cut one of those in half so that two arms remain, and mount it to a round brass wall plate. It's beyond cool.
As described here in the past, Louie's Cuisine of China in Ballard (zone just north of Seattle) has four VERY old Casa Victorians in the bar. AB, variable-speed, very shallow bell canopy, dark wood blades with open-weave cane (that's actually open, you can see the ceiling through the cane) with arch ends. These are in daily use. Their rental room has fifteen feet or so of barrel-vaulted plastic skylight containing a custom 70s Palmetto-type can with a lineshaft having three three-blade units with scoop-like woven rattan paddles. Ornate brass supports in pairs hold the lineshaft on top of cross-beams that have rows of light bulbs recessed in brass-lined coves underneath, and a short belt at one end connects to the motor hidden in the wall. If Louie's ever goes under (and I hope it doesn't, their food, service and decor are all awesome) I'll be asking the owner if I can buy the fans.

Does it count if the fan is one I installed? My uncle who owns George's Oxford Village, in Oxford, NC maps.google.com/maps/place?oe=utf-8&client=firefox-a&ie=UTF-8&q=george%27s+oxford+village&fb=1&gl=us&hq=george%27s+oxford+village&cid=17336006063470284467 added a bar onto his restaurant and I asked him when it was almost complete if he planned on putting a fan in the center as I had noted the Pendant lights were installed but there was a box left open, which I identified as a fan box. He said yes, but hadn't bought anything. Well, I happened to have this Oakleaf original laying around... so I rebuilt the thing, installed a 9mfd Cap for 'that' low speed and I put it up. This was just before Christmas. We turned one of the floods around over the bar and it shines down and casts the shadow of the ever so slowly spinning fan against the wall on the other side of the bar. It's pretty freakin' cool. I have a video as well. Apologize for the poor pictures, I lost the ones I took with the camera. The thing about it is though, the sconces on the wall, match the fan, the bar.. matches the fan and the blade finish matches the ceiling, looks like it was totally designed for the space, or, if you want to get all touchy feely.... that the space was designed around the fan. This is one fan that will indeed live a life of luxury. Running on low 24-7. I also put a 3 amp fuse on it... just in case ya know..

The ceiling height is 14ft at this point, so the long pullchain with the big ol' bushy tassel is pretty pointless, it just makes it so you can get to the pullchain with a chair. Next project? Two antique desk fans for on top of the bar, running through a variac.
